Using a Novel Spray-On Antimicrobial Hydrogel to Reduce Wound Bioload and Prevent Wound Bed Dehydration for Lower Extremity Chronic Wounds a Case Series

Background: Stabilized hypochlorous acid (HOCl) is a potent wound and environmental antimicrobial agent but short exposure times often limit its effectiveness. Recent studies have shown that HOCl achieved greater antibacterial efficacy against planktonic bacteria and biofilms as treatment times and irrigating volumes are increased. Most wound cleansers are liquids are applied by spraying or wiping the wound limiting their exposure time. Wound hydrogels that incorporate antimicrobials increase exposure time but application to wound surfaces can be difficult. Single-use tubes are expensive and the larger, more cost-effective tubes, are not designed for multi-use in our clinics. The absorptive and moisture-exuding properties of hydrogels make them ideal in prevention of wound desiccation and maintaining a wound environment conducive for auto lytic debridement and moist wound healing. A hydrogel incorporating HOCl that remains in contact with the wound bed would be very valuable.

Purpose: This poster evaluates a novel hypochlorous acid hydrogel that comes in a spray-on form that decreases risk of contamination from direct contact with the patient and provides multi-use functionality for economic value.

Methods: The viscosity of the gel formulation allows for horizontal or vertical layering of the application. It stays in place and does not drip off vertical surfaces minimizing waste. The spray-on feature facilitates easy, even application and access to deeper wounds with minimal patient discomfort. HOCl is a naturally occurring microbiocidal chemical produced by neutrophils in the body with both antipruritic and anti-inflammatory properties. 

Conclusions: Studies have shown HOCL to be highly reactive and effective against biofilm formation without causing cytotoxicity. Four cases of hydrogel application and follow-ups after several days of use are presented.
