A Novel Exit Strategy for Removal of a Mitraclip Device From the Left Atrium

Video 1. During retrieval the clip was inserted into the steerable guide catheter (SGC), but 1 clip arm remained wedged outside the SGC. Operators were unable to pull this back into the SGC and the arm became locked around the sheath.

Video 2. An attempt was made to retract the entire MitraClip delivery system (Abbott) across the interatrial septum into the right atrium, but the thick septum would not allow the system to exit the left atrium despite multiple attempts.

Video 3. A steerable sheath was positioned next to the MitraClip system (Abbott) and the push-pull technique was performed. The MitraClip system was retracted while forward pressure was applied with the steerable sheath to facilitate removal of the MitraClip system from the left atrium.
