Suicidal Behavior Among Patients With Psoriatic Arthritis

According to a study published in Seminars in Arthritis and Rheumatism, suicidal behavior in patients with psoriatic arthritis (PsA) was associated with depression and other psychiatric comorbidities.

Researchers aimed to assess the prevalence of suicidal behavior among patients with PsA and identify associated risk factors. Additionally, the study aimed to identify common genes or single nucleotide polymorphisms (SNPs) that may be involved in both suicidal behavior and PsA. Using the Preferred Reporting Items for Systematic Reviews and Meta-Analyses guidelines, the researchers conducted a systematic literature review, searching the databases PubMed/Medline, Web of Science, and EMBASE from their inception to May 2022. Full-text original articles that discussed suicidal behavior in patients with PsA were included in the analysis. The researchers also downloaded registered data from genome-wide association studies (GWAS) in the GWAS catalog database, focusing on PsA and psychiatric traits such as suicidal behavior and depression.

Out of 48 identified articles, 6 were relevant to the study question. Among a total of 122 160 patients with PsA included in the studies, 700 individuals exhibited suicidal behavior, representing a prevalence rate of 0.57%. Most patients with PsA and suicidal behavior were female, and the age range in 1 study was between 30 and 49 years. Suicidal ideation was reported in 0.53% of the 13 899 patients with PsA, whereas there were 125 suicide attempts (0.9%) and 13 completed suicides (0.07%) among 17 383 patients in 2 studies. The analysis also revealed a genetic haplotype on the chromosomal region 6p21.1, spanning from 29,597,596 to 32,251,264 Mb, which contained SNPs associated with both PsA and depression. This region includes the HLA genes of classes I, II, and III.

“Further evidence supports a genetic origin, at least partly. Awareness of these findings can help clinicians to recognize [suicidal] behavior and prevent suicide attempts,” the authors concluded.
