Development of a New Index for Psoriatic Arthritis Activity

According to a study published in Therapeutic Archive, the new Enthesial-Comorbidity Index of Psoriatic Arthritis Activity (ECIPsA), is an integral index for psoriatic arthritis (PsA) activity assessment, considering a spectrum of clinical parameters.

Researchers aimed to develop a comprehensive index for assessing PsA activity, integrating various clinical parameters. The study included 117 patients with PsA (63 men, 54 women) with a mean age of 44 years. Clinical examinations assessed PsA activity through joint counts, enthesitis, plantar fascia (PF) tenderness, skin lesion severity, presence of nail psoriasis, body mass index (BMI), erythrocyte sedimentation rate (ESR), C-reactive protein, Leeds Enthesitis Index (LEI) score, Disease Activity for Psoriatic Arthritis (DAPSA) score, and Functional Assessment of Chronic Illness Therapy-Fatigue (FACIT-F) score. Statistical analyses and a predictive model were employed.

Key findings revealed positive correlations between DAPSA score and several parameters, including BMI, age, ESR, PsA and nail psoriasis duration, tender and swollen joint counts, enthesitis, PF tenderness, skin lesion severity, and presence of nail psoriasis. Negative correlation was found between the FACIT-F score and male sex. Utilizing these parameters, the study proposed the ECIPsA, represented as 3.81×LEI + 13.72×PF + 0.54×Age - 0.25×FACIT-F + 7.36×Skin lesion severity + 7.94×PsA duration + 5.5×Nail psoriasis + 0.32×BMI - 3.52. The index demonstrated a correlation with high PsA activity when the DAPSA score was ≥28. Receiver operating characteristic analysis revealed the index's high correctness with an area under the curve of 0.768.

The new PsA activity index corresponds to the existing ones and takes into consideration the clinical heterogeneity and comorbidity of the disease,” the authors concluded.
