Changing Our Thinking About Cardiogenic Shock Patients: Hints From the Door-To-Unload STEMI Trial

Dee Dee Wang, MD, Henry Ford Medical Center, Detroit, Michigan

Deepak L. Bhatt, MD, MPH, Brigham and Women’s Hospital Heart & Vascular Center, Boston, Massachusetts

In this interview, Drs Bhatt and Wang discuss the Door-To-Unload (DTU)-STEMI pilot trial, set up to evaluate the safety and feasibility of left ventricular (LV) unloading and delayed reperfusion in patients with STEMI. The objectives were to determine the predictive value of cardiac magnetic resonance and echocardiographic parameters on LV remodeling in ST-segment elevation myocardial infarction (STEMI) patients without cardiogenic shock and treated with mechanical LV unloading followed by immediate or delayed percutaneous coronary intervention-mediated reperfusion.
