Aortic Rupture in TAVR

Introduction

Video supplement to "Aortic Rupture in Transcatheter Aortic Valve Replacement," by Blusztein and Agrawal. (July 2021 Clinical Images).

A 95-year-old man with severe symptomatic aortic stenosis was admitted for elective transcatheter aortic valve replacement (TAVR) to improve quality of life. During the TAVR procedure, the valve delivery system would not easily cross the aortic arch and the patient became distressed and hypotensive. Aortogram demonstrated large aortic injury distal to the left subclavian artery origin with significant extravasation. The patient did not survive despite cardiopulmonary resuscitation, rapid blood transfusion, and covered stenting of aortic injury. This case demonstrates a rare but catastrophic complication of TAVR.
