MitraClip XTR Implantation for a Calcified Leaflet

An 89-year-old man presented with dyspnea (NYHA class IV). He had undergone coronary artery bypass graft and surgical aortic valve replacement. The following imaging series demonstrates severe mitral regurgitation with flail anterior leaflet and calcified posterior leaflet. Subsequent treatment with transcatheter edge-to-edge repair using the MitraClip NTR clip; the result was suboptimal, so the device was retrieved and an XTR clip was implanted in the same position, with a good final result.
