2016 American Geriatrics Society Annual Meeting
The American Geriatrics Society (AGS) Annual Scientific Meeting is the premier educational event in geriatrics, providing the latest information on clinical care, research on aging, and innovative models of care delivery. AGS, a not-for-profit organization of nearly 6,000 health professionals, is devoted to improving the health, independence, and quality of life of all older people. The Society provides leadership to health care professionals, policy makers, and the public by implementing and advocating for programs in patient care, research, professional and public education, and public policy.
The 2016 Annual Meeting (#AGS16) addressed the needs and emerging insights of physicians, nurses, pharmacists, physician assistants, social workers, long-term care and managed care providers, health care administrators, and others committed to healthy aging. The Meeting was held in Long Beach, CA, May 19-21 and featured 100+ symposia, events, and networking opportunities focused on geriatrics research, education, policy, and health care delivery.

Advance Care Planning for LGBTQ Individuals and Older Adults with Dementia
One research poster at the 2016 AGS conference examined the largely unexplored area of how gay, lesbian, bisexual, or transgender identification influences patients’ priorities in advance care planning.
Decreasing Readmissions Through Transitional Care from SNF to Home
Presenters at the Conference conducted a cohort study using patients discharged from a local skilled nursing facility and found that the 30-day readmission rate dropped from 13.1% to 4% in the intervention group.
