Comparative Effectiveness of Two Collagen-based Dressings: Oxidized Regenerated Cellulose (ORC)/collagen/silver-ORC Dressing versus Standard Collagen Wound Dressing

Background: Complications in the healing of acute wounds may yield challenges to wound management and impact healthcare cost-containment.

Purpose: Here, we present a comparative effectiveness study to evaluate the value proposition of two collagen-based wound dressings: oxidized regenerated cellulose (ORC)/collagen/silver-ORC dressing and standard collagen wound dressing in matched cohorts of patients requiring the management of surgical or traumatic wounds.

Methods: Data extracted from the US Wound Registry identified surgical or traumatic wounds (n = 6055) treated with either ORC/collagen/silver-ORC (n = 5723) or standard collagen wound dressing (n = 332). Twenty-nine variables were used in propensity score matching to develop a case-matched cohort of 664 patients (n = 332 wounds per product group).

Results: Between the collagen dressings, the ORC/collagen/silver-ORC dressing group demonstrated a significantly higher percentage (56.3% vs. 39.8%) of wounds that healed (p<0.0001); The ORC/collagen/silver-ORC dressing group also demonstrated a significantly higher percentage of wounds that attained 75-100% granulation at zero depth at the 8-week (p = 0.0063), 12-week (p = 0.0010), 16- week (p = 0.0002), and 20-week (p = 0.0002) timepoints relative to the standard collagen wound dressing group. There was no difference in the number of dressing applications between the two cohorts. However, the duration of dressing use was significantly longer in the ORC/collagen/silver-ORC dressing group (12.8 days vs. 8.9 days [p=0.0210]) and there were more days between dressing changes (3.5 days v. 2.3 days [p=0.0012]).

Conclusion: According to our comparative effectiveness study employing real world data, ORC/collagen/silver-ORC dressing appears to afford improved healing and reduced time to granulation relative to standard collagen wound dressing.
