Angiography Demonstrating 2 Perforations in the LV Free Wall

A 93-year-old female patient with previous history of hypertension, atrial fibrillation, and ischemic stroke presented at the emergency department due to epigastric pain, malaise, sweating, and somnolence with 24 hours of evolution. This case illustrates a rare but catastrophic complication of acute myocardial infarction, ie, acute rupture of the left ventricular free wall.
