New Partnership Aims at Improving Care Coordination
In a recent announcement, NextLevel Health Partners, Inc, a Medicaid managed care company, said it would partner with PatientPing, a health technology company.
Together, the two companies will connect providers to seamlessly coordinate patient care. PatientPing will also work with NextLevel Health coordinators and inform them when a member has been admitted to a care facility. This will allow NextLevel Health to support safe discharge of their members.
"Safe hospital to home transitions are contingent on collaboration amongst healthcare providers," says Dr. Cheryl Whitaker, CEO of NextLevel Health. "This partnership will enable us to strengthen our care coordination efforts so that transition to home and back into the community based primary care loop has a chance to occur."
NextLevel Health, located in Illinois, offers a cloud-based health information technology system. The system allows users to see their health information, health profile, medications, doctor’s visits and tests, as well as obtain access to health education such as videos and articles. PatientPing, located in Boston, is a care coordination platform. The notifications that PatinetPing will provide to NextLevel Health coordinators will include context on a patient, including updated contact information. Notably, PatientPing’s network is made up of tens of thousands of providers nationwide.
