Congestive Heart Failure and Psoriasis

Dr Cohen explores the epidemiologic link between psoriasis and congestive heart failure (CHF), emphasizing the potential risks of TNF-alpha inhibitors in these patients. A randomized trial of infliximab showed no benefit in CHF patients, with higher doses linked to worse outcomes. Case reports also highlight new-onset or worsening CHF with TNF inhibitors. The discussion underscores the importance of avoiding TNF inhibitors in severe CHF (NYHA Class III/IV) and understanding the broader cardiovascular implications of psoriasis treatment.

Jeffrey M. Cohen, MD is a board-certified medical dermatologist and the director of the psoriasis treatment program at the Yale School of Medicine in New Haven, CT.
