Medical Management of Capgras Syndrome with Underlying Lewy Body Dementia in an Elderly Patient with Mild Cognitive Impairment

[3] Saint James School of Medicine, Saint Vincent and the Grenadines [1] Avalon University School of Medicine, Willemstad, Curaçao [2] Beckley Psychiatric Services, Beckley, West Virginia, USA [3] Abstract A case report to evaluate and discuss the medical management and subsequent interventions of an elderly patient with mild cognitive impairment, who presents with Capgras Syndrome and concurrent Lewy Body Dementia. An 89-year-old female with past medical history of Diabetes, Hypertension, DJD, CKD, CHF, Gout, Rheumatoid Arthritis, history of knee surgery, pacemaker placement, and hysterectomy with frequent periods of confusion and agitation presented with visual hallucinations. Our patient experienced extended periods of time in which she identified her husband as an imposter. Patient has no personal or family history of mental illness. It is important to note that this patient demonstrated intact memory and visual spatial recognition through evaluations with several Mini Mental Status Exams. This report will investigate the underlying pathology associated with both Lewy Body Dementia and Capgras syndrome that has been discussed to date, while also exploring methods of diagnosis and treatment interventions. The goal of this discussion is to emphasize the importance of identifying the association of Capgras syndrome with concurrent Lewy Body Dementia to provide effective medical management and understanding of the illness in order to limit emotional and psychological distress for the patient and family members or caregivers.
