To our knowledge, this is the first documented case of successful TAVI for severe quadricuspid aortic valve (QAV) stenosis performed in the U.S. and the first documented Sapien 3 valve implantation in a severely stenotic QAV.

This case highlights the value of extensive coronary collaterals in maintaining myocardial viability in severe coronary artery disease, and the role of cardiac MRI in guiding revascularization decisions.

At 6-month PCI follow-up, angiography showed patent left anterior descending and circumflex stents. However, OCT revealed almost all abluminal cavities were completely filled, which we surmise was due to a neointimal healing process.

A paravalvular leak (PVL) closure is often extremely challenging due to the leak’s small size and the fact that its location is not visible on X-ray during the attempted intervention.

The impact of contrast type on coronary imaging using optical coherence tomography (OCT) has received limited study. We compared OCT imaging obtained using the non-ionic, iso-osmolar iodixanol with the ionic, low-osmolar ioxaglate.

Spontaneous coronary artery dissection (SCAD) has been considered a rare cause of ACS in younger patients without known cardiovascular risk factors. However, recent studies using advanced imaging modalities indicate a higher incidence of SCAD...
