New Partnership Aims at Improving Care Coordination

February 2019

In a recent announcement, NextLevel Health Partners, Inc, a Medicaid managed care company, said it would partner with PatientPing, a health technology company.

Together, the two companies will connect providers to seamlessly coordinate patient care. PatientPing will also work with NextLevel Health coordinators and inform them when a member has been admitted to a care faciity. This will allow NextLevel Health to support safe discharge of their members.

“Safe hospital to home transitions are contingent on collaboration amongst healthcare providers,” says Cheryl Whitaker,  MD, MPH, CEO of NextLevel Health. “This partnership will enable us to strengthen our care coordination efforts so that transition to home and back into the community based primary care loop has a chance to occur.”

NextLevel Health, located in Illinois, offers a cloud-based health information technology system. The system allows users to see their health information, health profile, medications, doctor’s visits and tests, as well as obtain access to health education such as videos and articles.

PatientPing, located in Boston, is a care coordination platform that “connects health care providers with real-time clinical event notifications,” according to their website. The notifications that PatinetPing will provide to NextLevel Health coordinators will include context on a patient, including updated contact information. Notably, PatientPing’s network is made up of tens of thousands of providers nationwide.

“We’re excited that NextLevel Health is joining our growing Midwest community,” Jay Desai, CEO of PatientPing, said in a statement. “As a Medicaid MCO, NextLevel understands the value of identifying rising and high-risk members in real-time to ensure timely engagement and improved connectivity to community-based resources. Additionally, NextLevel Health can leverage our diverse provider network of PCPs & FQHCs to share important clinical context to inform care planning and treatment, ensuring member care is
truly coordinated between the plan and provider.”—Julie Gould
